Supplemental Waiver Information
If your insurance is comparable to the Oregon Tech Supplemental Insurance provided, you may qualify for a waiver. Go to Gallagher Koster to answer a few qualifying questions. You will need to create an account to access the online waiver.
Deadline for Insurance Waivers
Waivers will be available for Externs from June 18, 2012 thru July 13, 2012*
Waivers will be available for PEP, CLS, & RCP students September 14, 2012 - October 12, 2012*
On-line waivers, along with the required documentation*, will need to be completed and received in the ISHC by the dates below each term for your account to be credited:
Fall October 12, 2012
Winter January 18, 2013
Spring April 19, 2013
*You will need to send a copy of the confirmation email, a copy of your insurance card (front to back), and a copy of your insurance summary page to Cindy Childers. These copies can either be faxed to 541/885-1866 or emailed to cindy.childers@oit.edu.
Student accounts are credited the week before term begins, if all of your requirements are meet.
