Distance Education Application for Admission
Distance Ed offers programs in Allied
Health, Dental Hygiene, Diagnostic Medical Sonography, Echocardiography,
Information Technology, Operations Management, Polysomnographic Technology, Radiologic Science, Respiratory Care,
Technology and Management, and Vascular Technology. More information on these programs can be
found on the Distance Education web site under
Academic Programs.
Residents of some states are not eligible for admission into any of the online degree programs offered at Oregon Institute of Technology at this time. Click here for a list of those states.
For all of our online programs, residents of these states are not eligible for admission:
Alabama
Arkansas
District of Columbia
Kansas
Iowa
For our Polysomnographic Technology and Radiologic Science programs, residents of these states are not eligible for admission into these two programs.

It may be possible in the future to allow students from these states to
be admitted. Please contact us if you have questions regarding this
restriction or are interested in an online program at Oregon Tech but reside in

To apply for admission to one of our Distance Education programs, please complete the following steps:
1. Submit all required Oregon Tech documents, which are provided here in PDF format. Required documents include:
- Distance Education Application for Admission, including the Application Checksheet
- Statement of Acknowledgement (Note: Statements of Acknowledgement are not required for all programs. If one is required for your program, it is available here in PDF format.)
2. Submit all additional required materials as described on the Application Checklist (Admission Application page five). Your application will not be processed until all required materials are received. A transfer evaluation will be completed as part of the application process. For more information about this evaluation, please see the advising and registration page.
3. All documents should be mailed to:
